1. Interview Period: January 1, 2009 - June 15, 2009
   1. Interviews may actually take place before Jan 1, 2009.
   2. Applicants may attend SUFU Winter Meeting and AUA Annual Meeting and participate in the interview process.
   3. Applicants and Program Directors register with AUA and receive AUA Match ID number. (Deadline: May 15^th, 2009)
2. June 26, 2009: Applicant Rank list due to AUA
3. June 26, 2009: Fellowship Director Rank list due to AUA
4. July 14-21, 2009: Match results distributed to Applicants and Fellowship Directors.
5. Fellowship start date: July 1, 2010

1. Common Application Form available on SUFU website www.sufuorg.com. Applicant downloads copy of application form and completes application form and submits this form along with personal statement, curriculum vitae, and 2 letters of reference and mail to program directors of fellowships that one would like to interview with. The complete application will include:
   1. Application form (available on SUFU website as a PDF)
   2. Personal Statement
   3. Curriculum Vitae
   4. 2 Letters of Reference
2. Program Directors and Applicants must register for match with the AUA.

A link to the AUA website will be provided on the SUFU website. Each applicant must read and accept the terms and conditions of the AUA Match process, which are located on the AUA website. All Applicants are given a unique AUA Match ID number. Program Directors must also register their programs, and accept the terms and conditions of the AUA Match process, which are located on the AUA website. A unique AUA Match ID number will be applied to all programs. Applicants will send completed applications to the programs they desire. Contact information may be obtained in the SUFU Fellowship Directory on the SUFU website. It is the responsibility of the applicant to verify that applications have been successfully submitted to the desired programs, and that all requirements have been met.

3. Interviews will be arranged between the program directors and applicants.

Initiating interviews is NOT the responsibility of the AUA or SUFU. It is up to the applicant and program directors to insure that this process is complete.

4. Program Directors must submit complete, accurate contact information to

the SUFU website administrator. The contact person is Debbie Roller who may be reached at debbie@wjweiser.com. This information should be updated by November 1, 2008 to insure accuracy for the applicants.

1. The match is a binding agreement between applicant and program
2. The applicant should apply only to programs where he/she interested is interested in obtaining an interview.
3. Applicants should rank only programs in which they would be interested in attending. DO NOT rank programs that are not desired for future training.
4. Programs should only interview candidates after review of application and selection for interview.
5. An applicant's rank or position in the match is not to be discussed between applicant and program director.
6. Positions are NOT to be offered or inferred outside of the match process.
7. Violations of #5 and #6 will result in a program's removal from the match process. These conditions exist strictly to protect the applicant and program directors, and violations will not be tolerated.
8. Unmatched applicants and programs will be immediately released to the programs and applicants. This information will be transmitted to contact information provided by involved parties. It will be the responsibility of the applicant and program directors to initiate a post-match process.
